If you have spent any time on health content recently, you will have come across the terms 'cortisol face' and 'cortisol belly.' They have moved from wellness social media into mainstream search, as a lot of people recognise something in these descriptions. A face that looks puffier than it used to. Weight that has settled around the middle despite no obvious change in what you are eating.
These terms are oversimplified, and not everything that looks like cortisol accumulation is actually caused by cortisol. But they point to something real. Chronic stress does have physical consequences, and understanding the biology behind it is genuinely useful, both for making sense of what you are experiencing and for knowing what to do about it.
Cortisol is your primary stress hormone. It is released by the adrenal glands in response to perceived stress, whether that is a genuine emergency, a difficult conversation, a missed deadline, or chronic low-level pressure that never fully resolves. There is a second mechanism operating alongside the hormonal one, and it compounds the effect. Cortisol actively drives cravings for high-fat, high-sugar, calorie-dense foods, the evolutionary logic being that your body, believing it is under threat, wants to rapidly replenish energy stores. This is why stress eating tends toward biscuits and crisps rather than salad. The cortisol is doing its job; it is just not a job that serves you well in a modern context where the stress is a difficult inbox rather than a predator. Oestrogen has a moderating effect on the cortisol stress response. When oestrogen levels are stable, the HPA axis is somewhat buffered, the cortisol response to stressors is not completely suppressed, but it is blunted.
